As the City of Cagayan de Oro continues to promote social distancing among government-related transactions, residents can now process their medical certificate online. See below the process flow for online medical certificate:  

Step 1

Go to FB page: City Health Office Cagayan de Oro  

Step 2

Fill in this form https://bit.ly/MedicalCertificateCHO or send a message for instructions;  

Step 3

IF approved, an online appointment and priority number will be given;  

Step 4

During the scheduled time and date – present the original copies of the requirements for assessment.  

Note: Please allow 48hrs or 2 Business days for Processing Time

 

Requirements

APOR (Authorised Personnel Outside Residence)

1. Certificate of Employment (Within 30 days of issuance)
2. Company ID
3. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client
4. 1×1 recent photo (White Background)

  Non-LSI (Locally Stranded Individual)

1. Barangay Certification for Non PUM/Non PUI
2. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client
3. 1×1 recent photo (White Background)

  OFW

1. Barangay Certification for Non PUM/Non PUI
2. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client
3. Passport
4. 1×1 recent photo (White Background)

  Fit to Work (for Employment)

1. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client

  For LSI, you may process your medical certificate at the nearest Barangay Health Center.
