How to apply for DSWD Social Amelioration Program

As mandated by Republic Act (RA) 11469 or the “Bayanihan to Heal as One Act,” the Department of Social Welfare and Development (DSWD) announced Sunday that they will provide Social Amelioration measures to families belonging to vulnerable sectors good for two months. This financial assistance aims to support families affected by the community quarantine amid the coronavirus (COVID-19) crisis.

  Check out the steps below to learn how to apply for the Social Amelioration Package from DSWD.  

STEP 1

Authorized personnel from the local government will distribute the Social Amelioration Card (SAC) forms in their respective areas. The distribution will be on a house to house basis. To reiterate, please Stay At Home and wait for the officials to deliver the forms at your doorstep.  

STEP 2

The head of the family will be the one to sign up and fill in all the necessary information in the SAC form. The information must be complete and written in CAPITAL LETTERS. There will be two copies of the SAC form that need to be filled in.  

STEP 3

After filling up, submit one copy to the local government’s authorized personnel who will collect the forms. Meanwhile, the other copy must be kept by the household for future reference and monitoring.  

STEP 4

Once approved, the needed relief from the DSWD and other government agencies, such as the Department of Agriculture and Department of Labor and Employment, will be distributed through your respective local government unit. Note: The SAC form is a validation tool that aims to review families who need immediate assistance.
