Can’t find your vaccine information on the VaxCertPH system? If you live in Quezon City, here are three simple steps to follow as an alternative to obtaining your digital vaccine certificate.
UPDATE (December 22, 2021) — In accordance with the advisory the Department of Interior and Local Government (DILG) issued on December 4, Quezon City will only cater to Overseas Filipino Workers (OFWs) and individuals traveling abroad for the VaxCertPH issuance.
For domestic travel, the LGU-issued vaccine card continues to be considered valid.
Vaccine Records Not Found? 3 Steps on getting your VaxCertPH
Step 1. Before heading out, double-check the information you input on the system, including your personal information and vaccination details. Once done and yet the system still shows the ‘No Vaccination Record Found‘ message, proceed to the next step.
Step 2. Prepare a copy of your vaccination card, the one issued by your local government, and at least one (1) government-issued identification card (ID).
Step 3. Visit the City Hall or VaxCertPH booths located in any SM Mall branches. Bring the requirements mentioned in Step 2 and follow the instructions on site. Before leaving, make sure to leave your name, email, and mobile number and wait for approximately three (3) days to get your digital certificate.
