The lost passport process follows the new application process. All lost passport holders must present the following additional documents:

How to Apply for a DFA Passport?

To apply for a new Philippine Passport, applicants just have to follow these steps to set their appointments online. The Department of Foreign Affairs will not cater to walk-in applications. It is also important to mention that there is a fifteen calendar day clearing period for valid lost Philippine Passports.

STEP 1: Visit the DFA website and click Services.

STEP 2: Click on Passport, then Book an Appointment.

STEP 3: Click on Schedule an Appointment and check the box to begin.

STEP 4: Fill out the needed information.

STEP 5: Choose appointment date and time.

Applicants can then check for DFA online appointment slots, as shown in the images above for their DFA lost passport appointment. 

STEP 6: Make sure that all details are correct. Once confirmed, check the box to agree with the Department’s terms.

STEP 7: Fill out information correctly and accurately.

After filling out every section of personal information, ensure that all information is true and correct. Make sure to click ‘Next’ to proceed to the next section. 

Once applicants have filled out all relevant information, they can proceed to the next stage of the process – payment. Applicants have two payment options: normal and special processing.

Note: Special processing costs more. Make sure to get a passport early to avoid having to spend extra.

After choosing a processing type, applicants then select their chosen payment method. They can pay via e-wallets, online banking, or debit/credit card at designated payment centers. Here is a list of the accredited payment centers:

How to Replace Lost Valid Passports for Minors

The process and DFA lost passport requirements for minors is essentially the same as adults. However, lost Philippine Passports for minors require an Affidavit of Loss in English signed by parent/s, legal guardian, or person authorized by parent/s who will accompany the minor on the day of application. Overall, the lost passport of minors will also be treated as a new application.

Core Requirements for Minor Applicants

Below is a list of required documents for minor applicants of the Philippine Passport.

1. Confirmed online appointment
2. Accomplished application form
3. Personal appearance of minor applicant and either parent or authorized companion
4. Philippine Statistics Authority (PSA) birth certificate
   1. Local Civil Registrar Copy may be required if PSA birth certificate is not clear
5. Marriage certificate is required if only one parent is accompanying the child
   1. If parents are unmarried, a Special Power of Attorney (PSA) executed by the mother is required if she is not accompanying the minor
   2. If SPA is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
6. Passport or valid government-issued ID of either parent with one photocopy
7. School ID (if applicable)

Core Requirements for Passport Releasing

After completing all the DFA lost passport requirements, completing the online application for new applicants, and the personal appearance at the selected DFA office, applicants need to present the following documents for their new passports to be released:

1. Original DFA-issued receipt
2. Original acceptable ID
3. Old passport (for renewals only)

In the event that the applicant or authorized representative misplaces the original receipt they are required to show the Original notarized Affidavit of Lost Receipt executed by the applicant. 

Note: All unclaimed Philippine Passports will be canceled after a year per Department Order No. 2021-012.

In case the applicant is not able to claim their passport, immediate family of legal age can claim their passport with the following documents:

1. Letter of authorization (must mention relation to the applicant)
2. Original DFA-issued receipt
3. Valid ID (original and photocopy) of both applicant and authorized representative

However, authorized personnel outside the applicant’s immediate family cannot claim the passport, another individual can claim the passport with the following documents:

1. Original notarized Special Power of Attorney (SPA) executed by the applicant, issued to the authorized representative
2. Valid ID (original and photocopy) of both applicant and representative indicated in the Special Power of Attorney (SPA)

For minor applicants to claim their passports, the following are needed:

1. If claimed by the mother, claimant must present original DFA-issued receipt and valid ID
