  1. Application Letter stating the desired position, item number/code, salary grade and office/bureau/service/unit addressed to Director Leonardo C. Reynoso, CESO III of Human Resource Management and Development Service 
  2. Fully accomplished Personal Data Sheet (PDS) with recent passport-size picture (CS Form No. 212, Revised 2017) which can be downloaded at www.csc.gov.ph
  3. Duly accomplished Work Experience Sheet 
  4. Photocopy of certificate of eligibility/rating/license; and 
  5. Scanned Authenticated copy of Transcript of Record and/or Diploma; 
  6. Scanned Authenticated copy of Certificate of Grades for Master’s Degree earned units (If applicable); 
  7. Copy of Special Order or Certification indicating the supervisory/management experience/functions signed by the HRMO or any authorized representative (for Division Chief positions/if applicable);
  8. Copy of certificate of relevant trainings and seminars attended 
  9. For government personnel applicants: Certificate of No Pending Administrative, Civil, and/or Criminal Case (CENOPAC) For applicants from private companies: Certificate of Employment from the last/current employer 
  10. Scanned copy of NBI Clearance or proof of payment i.e. official receipt of the NBI clearance; and 
  11. Copy of Individual Performance Contract Rating (IPCR) in the last rating period or its equivalent for external applicants (Performance Evaluation/Appraisal) from the last/current employer.
